Aim to reduce the risk of homelessness and institutionalization among Non-Minor Dependents (NMD) aging out of Foster Care and Juvenile Court System.
MISSION
To provide least restricted safe housing with supportive services for non-minor dependents (NMD) in Foster Care and Juvenile Court System. Empower them with necessary tools and resources to build the basic skills they need to make a successful transition from care into self-sufficient and responsible adulthood.
Eligibility & Admission Requirements
Current foster youth ages 18 up to 21 in out-of-home placement with Child Welfare or Probation are eligible. For youth placed out of county, services are coordinated with the county where a youth is residing. To refer a youth, a Social Worker or Probation Officer is required to submit a referral form and the Transitional Independent Living Plan (TILP). Intake Application is online.
Independent Living Skills Classes
Provides classes for life skills training such as employment, education, housing, and money management. Financial incentives are paid to youth who decide to participate in the Love Focus Youth Outreach THPP-NMD.
